This reading interest survey was drawn from the article "But There's Nothing Good to Read" by Denice Hildebrandt. It is an example of a survey that probes teens to talk about their reading habits and their feelings about reading. The simple checklist asks about teens' interests in topics, books, libraries, favorite reading materials, and use of free time.
